Care does not end when an event concludes. If you were assessed or treated by our team, you should have access to your information and know who to contact if questions arise afterward.
Medical Record Requests
To request a copy of your medical record, please complete the form below with identifying information related to the encounter.
Requests are typically processed within 5 to 10 business days.
To protect patient privacy, records are only released to individuals with the legal authority to access them. Additional verification may be required before a request can be fulfilled.
All personal health information is handled in accordance with Ontario’s Personal Health Information Protection Act and is securely stored, accessed only by authorized personnel.
Clinical Follow-Up
If your condition required additional attention, a member of our team will reach out following your treatment to check on your status and ensure appropriate next steps have been taken.
For higher-acuity cases, this follow-up typically occurs within 7 days.
All higher-acuity cases are reviewed by a physician as part of our clinical oversight and quality assurance process.
